acceptance of my entry to the Shallow Sport of
Texas, Inc. Owners Tournament, I hereby agree that the Shallow Sport of Texas, Inc. Owners Tournament
Directors may, at their sole discretion, require
me to take a polygraph examination before a
polygraph examiner of their selection. I understand
that the decisions of the Directors and their polygraph
my right to bring legal action against Shallow
members, Tournament Committee members, agents,
volunteers, employees, or sponsors, including but
not limited to any and all polygraph examiners and
weighmasters employed by Shallow Sport of Texas,
Inc. to seek damages of any sort because of my being
I am aware of the rules of the Tournament and that I
enter at my own risk. Shallow Sport of Texas, Inc. and
any and all persons directly and indirectly connected
with the operation of the Tournament are exempt from
liability of loss, damage, negligence, harm & injury
which may occur before,during or after the Tournament. 
I here by give authorization to Shallow Sport of Texas, Inc. to use any and all photos taken during the tournament that might contain myself or my boat. This includes use in any and all publication produced by Shallow Sport Texas, Inc. website and promotional ads and brochures.
